HRT Patches in Spain: 2026 Shortage and Prescriptions

Can You Get HRT Patches, Gel or Spray in Spain?

Quick answer: Yes, HRT patches, gel and spray are available in Spain, but every form is strictly prescription-only. You cannot buy them over the counter at a pharmacy. Prescriptions from the UK or US cannot be dispensed directly by Spanish pharmacists. If you are staying in Spain or have run out of your regular supply, you need a valid prescription, such as a Spanish private prescription, to obtain your medicine.

To continue your HRT prescription in Spain, UrgentDoc connects you directly with English-speaking licensed doctors through a secure video consultation. The doctor reviews the HRT product, dose and form you currently take, evaluates your medical history, and determines whether continuing your treatment or adjusting to an available alternative is clinically suitable. If approved, your doctor issues an official Spanish private electronic prescription sent instantly to your email as a PDF, which you can take to a local pharmacy.

Do You Need a Prescription for HRT in Spain?

Many international visitors and residents ask whether they can buy HRT over the counter in Spain. The law is unequivocal. Every HRT product sold in Spain - patch, gel, spray, tablet, capsule or vaginal treatment - is prescription-only: in CIMA, the Spanish medicines agency's database, each is classed as 'Medicamento Sujeto A Prescripción Médica' (prescription-only medicine).

Under Spain's medicines legislation (Real Decreto Legislativo 1/2015), only a doctor, dentist or podiatrist can prescribe a prescription-only medicine, on a public or a private prescription, and a pharmacist may only dispense it against that prescription. The law classifies dispensing a prescription-only medicine without a prescription as a serious infringement, meaning Spanish community pharmacists are legally prohibited from selling estrogen patches, gels, sprays or tablets over the counter without a valid prescription.

Which HRT Patches, Gels and Sprays Are Marketed in Spain?

Women searching for their usual HRT brand or looking for online estrogen patch options in Spain often discover that the range of products differs from other countries. For instance, combined estrogen-and-progestogen patches are not available. As of 26 October 2026, no combined estrogen-and-progestogen patch is marketed in Spain. Combined HRT is available as tablets or a capsule, or as an estrogen product used with a separate progestogen.

According to Spain's menopause society (AEEM), many HRT products are approved in Spain, taken by mouth or through the skin and in different doses, and health professionals should use authorised products - "not just any mixture of hormones will do."

Is There a Shortage of HRT Patches and Sprays in Spain?

If you are trying to locate an HRT spray or find estrogen patches in Spain, you may encounter temporary stock issues. On 2 October 2026, the Spanish medicines agency (AEMPS) reported a shortage of several HRT medicines applied through the skin. It began in March 2026 with the only HRT spray sold in Spain, and the extra demand for the alternatives has caused intermittent shortages of some patches.

The agency notes that the spray's manufacturer expects supply to return gradually from November 2026 and into 2027; however, the AEMPS warns that its forecasts have not always been accurate.

As of 26 October 2026, CIMA listed active supply problems for five patch packs from two brands. For three of them, the agency's note says the prescribing doctor must decide whether other marketed treatments can be used.

The AEMPS explains that patches or gels can fill the gap left by the spray, and that estrogen tablets, alone or combined, can be used in some cases. Not all of these alternatives are funded by Spain's public health system. What can you do if you run out of HRT patches in Spain or find your spray is unavailable? While supply is uneven, you can phone the pharmacy to ask whether it has your form and strength in stock before you go. If your medicine is out of stock, a doctor can help identify an appropriate alternative form.

Can You Switch Between HRT Patches, Gel and Tablets?

When a particular patch or spray is unavailable, patients frequently ask whether they can switch to an estrogen gel or estrogen pills. Changing your HRT preparation or route of administration is a clinical decision that requires medical evaluation.

Guidelines from NICE, the UK's National Institute for Health and Care Excellence, point out clear physiological differences between routes of administration: NICE says the risk of blood clots is not increased with HRT through the skin, is increased with HRT tablets, and is greater with tablets than through the skin. It also says stroke risk is unlikely to increase with estrogen through the skin, but rises with estrogen tablets. Furthermore, NICE advises considering HRT through the skin rather than tablets for people at higher risk of blood clots, including those with a BMI over 30. That is why moving from a patch, gel or spray to tablets is a decision for the doctor, who weighs your history.

Your anatomy also dictates the structure of your treatment. If you still have your uterus, estrogen is combined with a progestogen; estrogen-only HRT is for people who have had a total hysterectomy, because estrogen alone can make the lining of the womb grow.

If you are experiencing supply problems or wish to discuss alternative administration routes, you can book an online HRT consultation to review your options with a licensed doctor.

Vaginal Estrogen Cream, Tablets and Rings in Spain

Some women require local treatment specifically for genitourinary symptoms. Vaginal estrogen is a separate, local treatment for vaginal dryness, pain with sex and urinary discomfort after the menopause. NICE says it can be used alongside HRT taken through the skin or by mouth, and that only a minimal amount reaches the bloodstream. Spain's menopause society calls it the treatment of choice for these symptoms.

According to Spain's menopause society (AEEM) and NICE recommendations, vaginal estrogen comes as a cream, gel, vaginal tablet, pessary or a low-dose ring, and as of 26 October 2026 every vaginal estrogen product in Spain is prescription-only. Because absorption into the bloodstream is minimal, NICE confirms that vaginal estrogen does not need to be combined with a progestogen to protect the womb lining.

If your primary concern involves local urogenital discomfort, you can schedule an online menopause consultation to evaluate whether a local vaginal estrogen prescription is appropriate.

Continuing HRT from Abroad: UK, US and EU Prescriptions

If you have relocated to Spain or are travelling here and find yourself running short of medication, navigating international prescriptions requires attention to Spanish pharmacy rules.

Under Spanish prescription regulations (Real Decreto 1718/2010), Spain's rule for prescriptions from other EU countries does not cover UK or US prescriptions, so you need a new prescription - for example, a Spanish one.

For EU citizens, a prescription issued in another EU country can be dispensed in Spain if the medicine is authorised here and the prescription carries the required details, though the pharmacist may still refuse if in reasonable doubt. According to official European Union healthcare rules, a medicine prescribed in one country might not be available in another, or may have another name.

When packing for travel, rules from the Spanish medicines agency (AEMPS) state that you may bring your own medicines into Spain. The amount should match your prescribed treatment and, as a rule, not exceed three months' supply; you may be asked for a prescription or medical report.

To ensure continuity of care, bring the box, the leaflet or your current prescription to the consultation, so the doctor can see the exact type, dose and form of HRT you use. Our service allows you to get your HRT prescribed online in Spain without delays. For women who have never used hormone therapy before, read our medical guide on starting HRT for the first time in Spain.

What to Expect After Changing Your HRT Form or Dose

Whenever a doctor adjusts your HRT dosage or switches your preparation (such as moving from a transdermal spray or patch to a gel), your body requires an adjustment period.

According to NICE guidelines, if you have a uterus, vaginal bleeding is a common side effect within 3 months of changing the dose or type of HRT, and in the first 6 months of starting it; NICE advises seeking medical help promptly for unscheduled bleeding outside those times.

Routine clinical follow-up is an integral part of ongoing hormone therapy. NICE recommends reviewing HRT 3 months after starting it and then once a year, or sooner if it is not working or causes side effects. At each review, the doctor evaluates whether the current dose and administration route remain effective and appropriate.

How a Video Consultation for HRT Works with UrgentDoc

UrgentDoc simplifies accessing medical care while in Spain. You book a video consultation with one of our licensed doctors at a time that suits you. The doctors speak English. Doctors send offers, and the consultation fee is shown on the doctor's offer before you choose a time and pay.

During the appointment:

  • For a woman already on HRT, the doctor reviews what she takes now and how it is working, takes her medical history, and decides whether to continue it, change the dose or change the form (for example if her usual form is out of stock).
  • For a woman new to HRT, the doctor assesses whether HRT is suitable.

A prescription is never guaranteed: it is always the doctor's clinical decision. If an in-person examination or specialist care is needed first, our doctors refer the patient for it.

What to have ready for an HRT consultation

ItemDetails to prepare
Current HRT detailsThe box, leaflet or current prescription of the HRT you use
Treatment historyHow long you have used it and how it is working
Health profileCurrent medicines and known allergies
Recent clinical measurementsA recent blood pressure reading and current weight
Reproductive historyThe date of your last period and whether you have had a hysterectomy
IdentificationA passport, DNI or NIE number to put on the prescription

If the doctor decides HRT is appropriate, you receive a private electronic prescription as a PDF by email, issued through a private e-prescription platform such as REMPe. Show the PDF at a pharmacy; the pharmacist checks it before dispensing. Under Spanish law, a Spanish prescription, public or private, is valid throughout Spain. The pharmacist checks it and will not dispense if there are reasonable doubts about its authenticity or validity. On the private route you pay two things: the consultation fee, shown on the doctor's offer before you pay, and the pharmacy's price for the medicine.

Our doctors do not prescribe HRT online, and refer instead, for:

  • Current or past breast cancer or another estrogen-dependent cancer
  • Unexplained vaginal bleeding
  • A past blood clot or a known clotting disorder
  • A stroke, heart attack or angina
  • Active liver disease
  • Menopause symptoms or periods stopping under age 40

Red Flags: When to Seek Urgent In-Person Care

Certain medical symptoms require immediate physical evaluation rather than a telehealth consultation.

According to the Spanish patient leaflet for an HRT gel, you must call 112 straight away if you identify any possible signs of a blood clot:

  • Pain and swelling in a leg
  • A sudden cough
  • Severe chest pain (sometimes also in the arm)
  • Difficulty breathing
  • A severe unusual headache
  • Loss of vision or double vision
  • Difficulty speaking clearly
  • Dizziness
  • Seizures
  • Weakness or numbness affecting a part of the body
  • Difficulty walking or holding things

You must stop your treatment and see a doctor immediately if you develop:

  • Yellowing of the skin or the whites of the eyes (which may indicate liver problems)
  • Swelling of the face, tongue or throat with difficulty swallowing, or hives together with difficulty breathing (with difficulty breathing, call 112)
  • A large rise in blood pressure
  • A first-ever migraine-type headache
  • Pregnancy

Regarding vaginal bleeding:

  • If you have a uterus, unscheduled vaginal bleeding is common within 3 months of changing the dose or type of HRT, and in the first 6 months of starting it; seek medical help promptly if it happens outside those windows, and see a doctor in person.
  • According to Spain's menopause society (AEEM), genital bleeding after the menopause that has not been explained needs an in-person assessment by a doctor, not an online appointment.

Frequently Asked Questions About HRT Patches, Gel and Spray in Spain

Do you need a prescription for HRT patches in Spain?

Yes. In Spain, only a doctor, dentist or podiatrist can prescribe a prescription-only medicine, on a public or a private prescription, and a pharmacist may only dispense it against that prescription.

Is there a shortage of HRT patches in Spain?

On 2 October 2026, the Spanish medicines agency (AEMPS) reported a shortage of several HRT medicines applied through the skin, which began in March 2026 with the only HRT spray sold in Spain and caused intermittent shortages of some patches. As of 26 October 2026, CIMA listed active supply problems for five patch packs from two brands. For three of them, the agency's note says the prescribing doctor must decide whether other marketed treatments can be used.

What can I use if my HRT spray or patch is out of stock in Spain?

The AEMPS says patches or gels can fill the gap left by the spray, and that estrogen tablets, alone or combined, can be used in some cases, although not all alternatives are funded by Spain's public health system. A doctor must evaluate your clinical history to determine whether switching to another form or brand is safe and appropriate. While supply is uneven, you can phone the pharmacy to ask whether it has your form and strength in stock before you go.

Can I switch from HRT patches to gel or tablets?

It is a decision for your doctor. NICE guidelines highlight that the risk of blood clots and stroke is not increased with HRT through the skin but rises with tablets, making transdermal forms preferable for individuals with higher risk factors such as a BMI over 30. If you switch preparations and have a uterus, remember that vaginal bleeding is common within 3 months of changing the dose or type of HRT.

Are combined HRT patches available in Spain?

No, not as of 26 October 2026. As of 26 October 2026, no combined estrogen-and-progestogen patch is marketed in Spain. Combined HRT is available as tablets or a capsule, or as an estrogen product used with a separate progestogen.

Can I use my UK HRT prescription in a Spanish pharmacy?

Spain's rule for prescriptions from other EU countries does not cover UK or US prescriptions, so you need a new prescription - for example, a Spanish one. A medicine prescribed in one country might not be available in another, or may have another name. Bring the box, the leaflet or your current prescription to the consultation, so the doctor can see the exact type, dose and form of HRT you use.

Can I buy vaginal estrogen cream without a prescription in Spain?

No. Vaginal estrogen comes as a cream, gel, vaginal tablet, pessary or a low-dose ring, and as of 26 October 2026 every vaginal estrogen product in Spain is prescription-only. You must consult a doctor to obtain a valid medical prescription before a Spanish pharmacy can dispense it.

How much HRT can I bring with me to Spain?

You may bring your own medicines into Spain. The amount should match your prescribed treatment and, as a rule, not exceed three months' supply; you may be asked for a prescription or medical report upon arrival or departure.
