Healthcare and prescription rules in Portugal

Portugal's Serviço Nacional de Saúde was restructured on 1 January 2024, when the five regional health administrations (ARS) were dissolved and replaced by 39 Unidades Locais de Saúde (ULS) — each one combining a hospital with the health centres around it under single management, so referrals and records sit in one organisation rather than two. Anything written about Portuguese healthcare before 2024 describes a structure that no longer exists. The autonomous regions sit outside this: Madeira is run by SESARAM and the Azores by their own regional service. For non-emergency symptoms the national nurse-triage line SNS 24 (808 24 24 24) assesses you by phone and directs you to self-care, a health centre or an emergency department; from 26 February 2026 calling it first became mandatory specifically for obstetrics and gynaecology emergencies, with exceptions where there is a suspected risk to life. EHIC and UK GHIC holders access state care on resident terms.

Antibiotics are classified as prescription-only in Portugal and cannot be bought over the counter. Amoxicillin, azithromycin, flucloxacillin, nitrofurantoin and the rest require a prescription from a licensed doctor, and pharmacies are supervised by INFARMED, the national medicines authority, together with the Ordem dos Farmacêuticos, including inspections and mystery-shopper checks on exactly this rule. Out of hours, a rota pharmacy — the farmácia de serviço — is posted at every closed pharmacy, and 112 is the emergency number.

  • Treatment for menopause symptoms under age 40; we refer you to specialist care for full evaluation
  • The hormonal coil (levonorgestrel intrauterine system), which requires in-person clinical insertion
  • Testosterone for women, as no testosterone product is licensed for women in Spain
  • Compounded 'bioidentical' hormones, as menopause societies advise against them; our doctors prescribe licensed medicines only
  • Progesterone skin creams or gels, not licensed in Spain for HRT or protecting the womb lining; our doctors do not prescribe them for that
  • Any medicine your medical history shows is not suitable or safe for you; we refer you to in-person care

Will a Spanish pharmacy accept a UK or US HRT prescription?

Under Spanish law, the rule allowing prescriptions from other EU countries to be dispensed does not cover prescriptions from the UK or the US, so you need a Spanish prescription. Additionally, medicine brand names often differ between countries, and some foreign products have no Spanish equivalent. For example, no combined estrogen-and-progestogen patch is sold in Spain. You should bring details of your current HRT, and our doctors can discuss an alternative available in Spain.

Can I start HRT online for the first time?

You can have an initial assessment online, but no menopause guideline endorses starting HRT through an online consultation. In Spain, product information states that doctors must take a full personal and family medical history before starting treatment. Our doctors evaluate this by video consultation and decide whether an in-person examination, such as a breast or pelvic check, is required first. Certain health conditions always require in-person or specialist care rather than remote care.

How do HRT renewals work?

To renew your HRT, have the name, strength and dose of your current medication ready for your consultation. Our doctors will review your symptoms, health history and blood pressure to assess whether the treatment remains suitable for you. Clinical guidance advises reviewing HRT after about 3 months when starting or changing a dose, and at least once a year thereafter. If continuing treatment is clinically appropriate, a private electronic prescription is sent to you as a PDF by email to present at a pharmacy.

Do I need blood tests for HRT if I am over 45?

In women aged 45 and over, menopause is usually recognised from symptoms and menstrual changes without routine hormone blood tests. During your video consultation, our doctors review your medical history, blood pressure and weight, and may request blood tests if your history calls for them. However, if the non-hormonal medicine fezolinetant is considered, liver blood tests are required before starting treatment and must be repeated monthly for the first three months.

Is HRT a contraceptive?

No, HRT is not a contraceptive method and cannot be relied on to prevent pregnancy. If you are sexually active and wish to avoid pregnancy, you still need effective contraception. Clinical guidance advises continuing contraception for 2 years after your last natural period if you are under 50, and for 1 year if you are over 50. Most women can stop at age 55. Withdrawal bleeds on HRT, or having no bleeding on HRT, do not count as natural periods, so agree with the doctor when to stop, continuing until 55 if in doubt. If you use hormonal contraception, ask our doctors during your consultation for guidance.

How much does it cost?

The consultation fee is clearly shown on the doctor's offer before you choose an appointment time and pay. If a prescription is clinically indicated and issued, the cost of the prescribed medicine is completely separate from our consultation fee and is paid directly to the dispensing pharmacy.

When do I need urgent or in-person care?

Bleeding is common in the first 6 months of HRT and within 3 months of a change. You should arrange an in-person medical examination if you experience unexpected bleeding that is heavy, continues beyond those windows, or starts after a bleed-free spell, bleeding after your periods have stopped for a year, or a new breast lump. If you develop possible signs of a blood clot or stroke—such as sudden chest pain, shortness of breath, a painful swollen leg, sudden weakness or numbness on one side of the body, or difficulty speaking—call the emergency services on 112 immediately.
