Funchal is the one location on this list not covered by the Portuguese SNS at all. Madeira is an autonomous region with its own health service, SESARAM (Serviço de Saúde da Região Autónoma da Madeira), managed by the regional government rather than Lisbon — the 2024 ULS reform applied to mainland Portugal and did not change Madeira's structure. Practically, that means the mainland's referral routes and some of its digital services do not map directly onto the island, though EHIC and UK GHIC holders still access state care on resident terms and pharmacies operate the same farmácia de serviço rota. Madeira is an Atlantic island roughly 1,000 km southwest of Lisbon, so onward transfer and repatriation are flights with real lead times. Its tourism is year-round rather than seasonal, and levada walking and the island's very steep terrain generate a constant pattern of falls, sprains and ankle injuries.
